This is a very obscure truck, but now we can say that both George Murphey and John Frances have found the answer. This is a Garnett, built by the Garnett Truck and Coach Company of Denver, Colorado.  E.V. Garnet thought he had a better idea about the trucks he needed for his Colorado Transit Company, so he built them himself. His special dual wheel front axle was rated at 20,000 lbs. Garnett built these trucks for at least 5 years, according to this January 1941 article from CCJ. Thanks to Bill White and especially George Murphey for tracking this rare truck down.

EV Garnett is the same guy who built the front axle for this truck. He was the president of Truck Equipment Co. He patented the dual front wheel stuff in the late '30s here, here and here. WAI on the Gallagher truck here.
